Guest Acknowledges and accepts the risks inherent in the use of the club or its facilities. By use of Impacts facilities and services, the guest hereby voluntarily assumes the risks of injury, accident, death loss, cost or damage to personal property which might arise from the use of Impact Athletics services or facilities. The guest, heirs, executors, representatives or assigns, hereby release Impact Athletic, its owners and employees for claims, liabilities for personal injury or property damage of any kind sustained by the Guest while on the premises. Guest further certifies that he or she is in good physical health and able to undertake and engage in the physical exercise or sports activities in which he or she chooses to participate.By signing below I acknowledge that I have read and understand the release above. If I am signing for my minor I understand that by signing this release I am giving permission for my minor to be at Impact Athletic and to participate in the activities there. This release will remain in force for each visit to the Club until the minor turns 18 and can sign for him/herself. If I am signing for myself it will it will stay on file and be checked each time you use the facility. If at anytime our employees are unable to pulI up your release another one will need to be filled out.
